Beta Arteether also called Artemotil is a quick acting blood schizonticide, exclusively prescribed for the management of chloroquine-tolerant Plasmodium falciparum malaria and brain malaria types. It is a quasi-synthetic derived of artemisinin, a natural production from the Chinese plant Artemisia annua or sweet wormwood. It is presently employed as a secondary treatment option for serious instances of malaria.

Mechanism of Action

As mentioned earlier, alpha or beta Arteether acts effectively and quickly as a blood schizonticidal agent in case of P. falciparum malaria at the erythrocytic level. It is saturated in parasitized erythrocytes (fully grown blood cells that comprise hemoglobin to transmit oxygen to the physical tissues). The operational faction accountable for anti-malarial action of α / β Arteether is endoperoxide bridge. Iron from the concentrated hemoglobin of the parasite's sufferer brings down this bridge, discharging an extremely responsive free radical iron group that stimulates lysis (dissolution and destruction) of the parasitical cell.

As destruction of parasitic cell membrane takes place, some significant damages occur, including puffiness and malformation of food vacuoles membrane, nuclear tissue layer, endoplasmic reticulum and creation of autophagic cavities. It is also projected that α / β Arteether stamps down the protein synthesis and modifies the ribosomal constitution and endoplasmic network of cells.

Dosage & Administration
Arteether is intended for intramuscular (within a muscle) application only. For adults, the recommended dosage is 150 mg that means 1 vial of Arteether at one time everyday for 3 successive days. In case of children, the prescribed dose is 3 mg/Kg for every day dispensed by an injection into a muscle throughout a 3-day period.
The injection should be administered under sterile conditions, deeply in the higher lateral quarter-circle of the buttock in an intramuscular way. No other medication should be blended in the same syringe.

Precautions

Ample researches involving harmless usage of artemisinin derivatives for the duration of pregnancy do not exist. Artemisinin derivatives like Arteether must not be taken in pregnancy as chief medications for simple malaria cases, but these can be applied for intervention of life-threatening or complicated Plasmodium falciparum malaria infection in sufferers of several drug resistances, if the likely benefit gives good reason for the potential danger to the unborn baby.

It is not acknowledged whether α / β Arteether is released into breast milk. Since many medications are passed into breast milk, carefulness should be implemented while using this drug. Arteether injection is contraindicated in patients who have known allergy artemisinin derivatives.

Side Effects

While toxicity to nerves or nerve cells has been told about in observational animals, there is no proof of this type of toxicity in people with artemisinin derivatives. Arteether generally goes down well with most patients. Nevertheless, nausea, lightheadedness and dejected GIT action can happen. Medical, neurological, electrocardiographic and biochemical supervising did not bring out noteworthy toxicity. Except for some addition in eosinophil (a leukocyte promptly stained with eosin) counts, no haematological deformity was witnessed.
